Double heterologous IVF-ET entails the use of a male donor of semen and a female donor of ovules. This technique is indicated in the case of both male and female infertility.

The double heterologous IVF-ET technique includes a phase of preparation of the uterus for the woman in order to allow for accepting the embryo fertilised with the male donor’s semen. The oocytes coming from the female donor are fertilised in vitro with the male donor’s spermatozoa, after which the embryos are transferred into the patient’s uterus.

This procedure is indicated in specific cases of female infertility (depletion of the ovarian function, early physiological menopause, surgical menopause with removal of the ovaries, genetic diseases transmittable to offspring, repeated failures with other assisted fertilisation techniques, repeated failures of embryonic nesting, and endometriosis), and serious male infertility.

IN DETAIL

1 Selection of the female donor
and start of the ovarian stimulation procedure with ovocytary pickup and cryopreservation of the ovules.

2 Selection of the male donor.

3 Preparation of the recipient female
the patient takes hormones either orally or transdermally (by means of patches) so that her endometrium is ready to receive the embryos.

4 In vitro insemination
the female donor’s oocytes are fertilised with the selected male donor’s semen and prepared for fertilisation. After about 20 hours the development of the zygotes is ascertained (pre-embryonic phase). The zygotes destined to be transferred into the uterus are left in cultivation for another 24-48 hours where they become real embryos. The average fertilisation rate is approximately 70%.

4 Embryo transfer
the embryos are transferred into the patient’s uterine cavity by means of a thin catheter. The operation is quick, painless and only lasts a few minutes. Normally only one or two embryos are transferred to avoid the risk of multiple pregnancies.

HOW
With ovule donation, the oocytes from a female who is compatible with the recipient female are fertilised in vitro with the semen of the male donor after which the embryos are transferred into the uterus of the female recipient.

WHEN
The treatment is carried out in 5 steps: a female donor compatible with the recipient female is selected and subjected to ovarian stimulation and subsequent ovocytary pickup; in the meantime the endometrium of the recipient female is prepared in order to be ready to receive the embryos. The oocytes are fertilised in vitro with the male donor’s semen. After 3-5 days the embryos are transferred into the recipient female’s uterus by means of a thin catheter in a quick and painless procedure.

COSTS
